Introduction to division

Although a community-based integrated care system is being promoted in recent times, there are still many issues that must be addressed in the division of home care. In order to fulfill the desire of people who "want to live in the place where I am used to living/where I want to live," it is necessary to know the needs of each patient and to consider, practice, and evaluate the methods of providing the care that meets their needs. In this division, we conduct research related to "home care nursing" and "support to transition from hospital to home" focusing on discharge planning, outpatient care, and providing care through home care nursing agencies.

Message

We have supported a wide range of research topics related to home care up to now, from those on the children to older who need medical care. We will continue to collaborate with the Keio University Hospital and home care nursing agencies. We are also involved in a research project on community activities of residents in Fujisawa City. We will be happy to work with you individually to develop a research topic that you want to study, so if you are interested in this division, please contact us.
